Adopters Acknowledgement of Terms
The Adopter(s) agree to assume full responsibility and guardianship of

Adopter agrees to provide all medical care including an annual exam, all vaccinations and monthly heartworm, flea and tick prevention.

Adopter agrees to provide a living condition which includes nutritional food, clean water, and a proper amount of exercise for the duration of their life. The dog or cat is to be left indoors, not to be left tied outside.

Adopter understands that HLAR has little knowledge of the animalās previous medical condition/background and disposition. HLAR makes no warranties with regard to the health of the animal and disclaims any liability for the animalās medical condition, sickness or disease.

Adopter releases Happy Life Animal Rescue and volunteers, foster homes and the veterinarian treating the animal at the request of HLAR from any and all claims/action, whether for property damage caused by the animal in foster care or for personal injury to Adopter, family member(s) or other animals in Adopterās care (personal pets or livestock) from biting, scratching, transmittal of disease, and or any other matter attributed to said animal(s).

Adopter agrees if any behavioral issues should arise that they seek a professional behaviorist for assistance, lessons or training. Adopter agrees to make every effort to correct any bad behavior. Animals are NOT permitted to be dropped off at adoption locations.

The Adopter agrees to pay HLAR an adoption fee of $425.00 for dogs and $200 for cats, plus charge fee if applicable. The adoption fee is non-refundable.

The Adopter agrees to pay all transportation costs, if applicable. The transportation cost is non-refundable.

The Adopter agrees this pet will be examined within 2 to 3 days of the adoption date by a licensed veterinarian and to immediately activate the 1 month trial pet insurance. All charges related to the veterinarian visit will be the responsibility of the Adopter. The adopter agrees to have this pet on heartworm prevention and flea/tick preventions.

While HLAR makes every effort to place only healthy animals, with good temperaments and makes every effort provides accurate history and assessment of the pet, HLAR cannot guarantee the health, age, breed, medical status, behavior or disposition of any animal and shall not be held responsible or liable for any medical expenses that may be incurred after the date of adoption. HLAR makes no explicit or implicit guarantees in any of the above. The adopter accepts the pet āas isā and assumes all responsibility for treatments of any and all conditions of physical or temperment changes that may occur.

The Adopter understands that the pet being adopted comes from a shelter environment and may have been exposed to worms,parasites, kennel cough or other unknown diseases. These are very treatable and should not be cause for concern. Worming protocols are started as part of the medical regime HLAR provides when first receiving the pet, but due to the life cycle of different parasites it may take several more treatments to ensure all forms of any internal parasites have been killed. HLAR does not cover any illness your new pet may get. The parties hereby acknowledge that HLAR has not made any warranties relating to the pet, either express or implied without limitation, any warranties regarding health, temperament or whether the pet is housebroken.

In consideration of the receipt of the referenced pet herein, the Adopter hereby understands, promises and agrees to forever release, discharge, indemnify and hold harmless HLAR, its directors, officers, volunteers and successors and assigns from any and all claims, damages, costs, expenses, loss of service, action and causes of action, including attorneyās fees arising out of any act or occurrence relating to the referenced pet herein from the present time forward.

Attached hereto and incorporated herein so as be inclusive in this adoption contract is a copy of the online application for set adoptions completed by adopter. Adopter has truthfully responded to all inquiries on the application and agrees to abide by all of these terms.

Adopter agrees to the Terms and Conditions within, HLAR may take appropriate action, including taking the pet from Adopterās possession and/or take legal action, in the event Adopter fails to comply with the above, or puts the animal in a situation which endangers health in any way.
